
Decentralized Clinical Pharmacist
The Children’s Hospital of Philadelphia is looking for a highly motivated Decentralized Clinical Pharmacist to join our team and help provide the highest quality of care. This position requires a PharmD degree and a minimum of two years of clinical experience. Responsibilites:
- Work collaboratively with healthcare teams to ensure the highest quality of patient care.
- Provide appropriate medication therapies and interventions for patients.
- Monitor patient responses to medications and adjust accordingly.
- Maintain patient medication profiles and provide accurate, timely documentation.
- Educate patients and families on appropriate medication use.
- Act as a resource to other healthcare providers regarding drug therapy and pharmacy services.
- Participate in quality improvement initiatives and research projects.
- Stay current on new medications, literature, and regulations.
- Comply with all applicable laws and regulations.
- Maintain patient confidentiality and adhere to HIPAA guidelines.
