Job Description

The Children’s Hospital of Philadelphia is looking for a highly motivated Decentralized Clinical Pharmacist to join our team and help provide the highest quality of care. Our ideal candidate will have experience in a clinical setting, a passion for pediatric care, and a dedication to improving patient outcomes. This position requires a PharmD degree and a minimum of two years of clinical experience. We are looking for a pharmacist who is committed to patient safety and is comfortable working in a fast-paced environment. The successful candidate will need to be a strong communicator, have excellent problem-solving skills, and be able to work collaboratively with healthcare teams. We are seeking a Decentralized Clinical Pharmacist who is passionate about providing the best care for our patients and families. Responsibilites:

  1. Work collaboratively with healthcare teams to ensure the highest quality of patient care.
  2. Provide appropriate medication therapies and interventions for patients.
  3. Monitor patient responses to medications and adjust accordingly.
  4. Maintain patient medication profiles and provide accurate, timely documentation.
  5. Educate patients and families on appropriate medication use.
  6. Act as a resource to other healthcare providers regarding drug therapy and pharmacy services.
  7. Participate in quality improvement initiatives and research projects.
  8. Stay current on new medications, literature, and regulations.
  9. Comply with all applicable laws and regulations.
  10. Maintain patient confidentiality and adhere to HIPAA guidelines.
