
Decentralized Clinical Pharmacist
.We are looking for an experienced Decentralized Clinical Pharmacist to join our team at the Children's Hospital of Philadelphia. The ideal candidate will possess a strong background in clinical pharmacy services with a passion for providing safe and quality care to pediatric patients. Our Decentralized Clinical Pharmacist must have a Doctor of Pharmacy (Pharm.D.) degree, or equivalent, and a minimum of two years of experience in clinical pharmacy services. Certification in Pediatric Pharmacy is strongly preferred. The successful candidate must also possess excellent interpersonal skills and be able to work independently in a fast-paced, dynamic environment. The Decentralized Clinical Pharmacist will be an integral member of our healthcare team and will be responsible for providing direct patient care, monitoring drug therapy, and delivering educational services to our pediatric patients. Responsibilities:
- Provide direct patient care for pediatric patients.
- Monitor drug therapy and provide clinical pharmacy services.
- Deliver educational services to patients and families.
- Develop and implement drug protocols and guidelines.
- Participate in interdisciplinary team meetings to review patient care and progress.
- Research and review new medications and treatments.
- Maintain current knowledge of industry trends and standards.
- Perform quality assurance activities and develop performance improvement plans.
- Develop and maintain collaborative relationships with patients, families, and other healthcare providers.
- Prepare and deliver reports and presentations.

Bachelor's Degree In Pharmacy
Registered Pharmacist In The State Of Pennsylvania
At Least Three Years Of Experience In A Pediatric Clinical Pharmacy Setting
