
Health Navigator
Children's Hospital Colorado is looking for an experienced, compassionate Health Navigator to join our team and help families navigate the healthcare system. As a Health Navigator, you will use your expertise and training to provide support, information, and resources to patients and their families. This is a unique opportunity to work in a highly respected and collaborative environment while making a significant impact in the lives of children and families.The ideal candidate for this position will have an advanced degree in a health-related field and previous experience working with families, children, and healthcare systems. You must possess excellent communication skills, both verbal and written, as well as strong problem-solving and interpersonal skills. Responsibilities:
- Provide support, information, and resources to patients and their families in order to guide them through the healthcare system.
- Utilize expertise and training to assess the healthcare needs of patients and their families.
- Develop and maintain relationships with patients and families to ensure their needs are met.
- Document patient and family interactions in order to build an effective care plan.
- Collaborate with internal teams and external stakeholders to ensure patient and family needs are met.
- Remain up-to-date on current healthcare policies and procedures.
- Ensure compliance with all relevant regulations and laws.
- Maintain confidentiality of all patient and family information.
- Monitor patient and family feedback in order to continually improve services and care.
- Develop and implement strategies to increase patient and family satisfaction.
