As a Family and Peer Support Partner, you will play a vital role in providing emotional and practical support to our patients and their families during their time at the hospital.   1. Provide emotional support to patients and their families during their hospital stay.
  2. Deliver practical support to patients and their families, including helping with daily tasks and providing resources and information.
  3. Serve as a mentor and role model for patients and their families, sharing your own experiences and providing guidance and encouragement.
  4. Collaborate with hospital staff to identify and address the unique needs of each patient and their family.
  5. Develop and implement support plans for patients and their families, taking into account their individual goals and needs.
  6. Facilitate support groups and educational programs for patients and their families.
  7. Maintain strict confidentiality and adhere to all hospital policies and procedures.
  8. Act as an advocate for patients and their families, ensuring their voices are heard and their needs are met.
  9. Participate in training and professional development opportunities to enhance your skills and knowledge.
  10. Continuously evaluate and improve the support services provided to patients and their families.
