- Conduct comprehensive reviews of patient medical records to assess the appropriateness and effectiveness of antimicrobial therapy.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized treatment plans for pediatric patients.
- Provide educational resources and support to patients and their families on proper medication use and potential side effects.
- Participate in multidisciplinary rounds and meetings to discuss patient care and make recommendations for optimizing antimicrobial therapy.
- Stay current on the latest research and developments in antimicrobial therapy and share this knowledge with the healthcare team.
- Monitor and assess patient responses to antimicrobial therapy and make necessary adjustments as needed.
- Ensure compliance with hospital policies and procedures, as well as state and federal regulations, regarding medication use and dispensing.
- Communicate with pharmacy staff and other healthcare professionals to ensure accurate and timely medication administration.
- Maintain accurate and complete patient records, including medication histories and adverse drug reactions.
- Assist in the development and implementation of antimicrobial stewardship initiatives to promote the appropriate use of antibiotics.
- Participate in quality improvement initiatives to enhance patient outcomes and reduce medication errors.
- Serve as a resource for medication-related inquiries and provide guidance to pharmacy staff and other healthcare professionals.
- Maintain a safe and clean work environment and adhere to infection control protocols.
- Act as a role model for professional behavior and maintain a positive and collaborative attitude towards colleagues and patients.
- Continuously strive for personal and professional growth by attending educational seminars and conferences.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy.
Board Certification In Pharmacotherapy (Bcps) Or Infectious Diseases Pharmacy (Bcidp).
Residency Training In Infectious Diseases Pharmacy Or Equivalent Experience.
Experience In Pediatric Pharmacy Practice And Antimicrobial Stewardship Programs.
Knowledge Of Antimicrobial Resistance Patterns And Evidence-Based Practices For Antimicrobial Use In Pediatric Patients.
