- Review and assess patient medication orders to ensure accuracy, appropriateness, and safety for pediatric patients.
- Collaborate with physicians, nurses, and other healthcare professionals to develop comprehensive treatment plans for pediatric patients.
- Provide drug information and education to patients, families, and healthcare providers.
- Monitor medication therapy and make recommendations for dose adjustments, drug interactions, and potential adverse effects.
- Prepare and dispense medications in accordance with legal and ethical standards.
- Conduct medication reconciliation for pediatric patients during admission, transfer, and discharge.
- Participate in medication safety initiatives and quality improvement projects to enhance patient care.
- Stay updated on the latest developments in pediatric pharmacy and implement evidence-based practices.
- Maintain accurate and complete patient medication records.
- Comply with all pharmacy laws, regulations, and standards of practice.
- Train and supervise pharmacy technicians and interns.
- Maintain inventory of medications and communicate with suppliers to ensure timely and adequate supply.
- Serve as a resource for medication-related questions and concerns from healthcare team members.
- Adhere to hospital policies and procedures related to medication management and safety.
- Provide excellent customer service to patients, families, and healthcare providers.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.
Current Pharmacist License In The State Of Colorado.
Board Certification In Pediatric Pharmacy (Bcpp) Or Equivalent Specialty Certification.
Minimum Of 2 Years Of Clinical Experience In Pediatric Pharmacy.
Strong Knowledge Of Pediatric Medication Dosing, Drug Interactions, And Formulary Restrictions.
