
Behavioral Health Care Coordinator

- Coordinate and facilitate mental health services for children and families in need.
- Conduct initial assessments and screenings to determine appropriate treatment plans.
- Collaborate with multidisciplinary team members to develop and implement individualized care plans.
- Communicate and coordinate with outside providers, schools, and community resources to ensure comprehensive care for patients.
- Monitor and track patient progress and make adjustments to treatment plans as needed.
- Provide education and support to patients and families regarding mental health resources and coping strategies.
- Maintain accurate and timely documentation of patient interactions and treatment plans.
- Participate in team meetings and case conferences to discuss patient progress and develop treatment goals.
- Advocate for patients and families to ensure their needs are met and their voices are heard.
- Stay up-to-date on best practices and advancements in the field of behavioral health to provide the highest quality care.
- Adhere to all ethical and legal standards and maintain patient confidentiality.
- Foster a compassionate and supportive environment for patients and families.
- Collaborate with insurance providers and assist with insurance authorizations as needed.
- Attend trainings and professional development opportunities to enhance skills and knowledge.
- Uphold the mission and values of Children's Hospital Colorado in all job duties.
Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or Related Field.
Minimum Of 2-3 Years Of Experience In Behavioral Health Care Coordination Or Case Management.
Knowledge Of Mental Health Diagnoses, Treatment Options, And Community Resources.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Multidisciplinary Teams And Communicate Effectively With Patients And Families.
Experience Working With Pediatric Populations And Understanding Of Child And Adolescent Development.
