Job Description

We are currently seeking a compassionate and experienced Behavioral Health Care Coordinator to join our team.As a Behavioral Health Care Coordinator, you will play a crucial role in ensuring the well-being of our patients by coordinating their mental health care needs. You will work closely with our interdisciplinary team to develop and implement treatment plans, provide support and resources to families, and advocate for our patients' mental health needs.The ideal candidate for this position will have a strong background in behavioral health, excellent communication and organizational skills, and a passion for working with children and families.   1. Develop and implement individualized treatment plans for patients in collaboration with the interdisciplinary team.
  2. Coordinate and monitor mental health services for patients, including therapy sessions, medication management, and crisis intervention.
  3. Conduct assessments and screenings to identify mental health needs and develop appropriate interventions.
  4. Work closely with patients and their families to provide education, support, and resources related to mental health care.
  5. Advocate for patients’ mental health needs and collaborate with community resources to ensure continuity of care.
  6. Maintain accurate and timely documentation of patient progress, treatment plans, and services provided.
  7. Collaborate with other healthcare professionals and outside agencies to provide comprehensive care for patients.
  8. Participate in multidisciplinary team meetings to discuss patient progress and make recommendations for treatment.
  9. Conduct outreach and education initiatives to raise awareness about mental health and available resources.
  10. Keep up-to-date with current research and best practices in the field of behavioral health to improve patient care.
  11. Maintain confidentiality and adhere to ethical standards in all aspects of the job.
  12. Demonstrate a compassionate and empathetic approach to working with patients and their families.
  13. Communicate effectively with patients, families, and colleagues to ensure a positive and collaborative work environment.
  14. Adhere to all hospital policies and procedures related to patient care, safety, and confidentiality.
  15. Participate in ongoing training and professional development opportunities to enhance skills and knowledge in the field of behavioral health.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Or A Related Field.

  • Minimum Of 2 Years Experience Working In A Behavioral Health Setting, Preferably With Children.

  • Knowledge And Understanding Of Mental Health Disorders And Treatment Approaches.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Healthcare Professionals.

  • Ability To Multitask, Prioritize, And Work In A Fast-Paced And Dynamic Environment While Maintaining Attention To Detail And Accuracy.
