We are looking for an exceptional Registered Nurse Care Coordinator to join our team and help us continue to provide the best care possible. The ideal candidate for this role will have a thorough knowledge of nursing processes, superior communication and interpersonal skills, and an understanding of the importance of delivering a high standard of patient care. We are looking for someone who is proactive, organized, and motivated to help us build a strong and compassionate healthcare team.The Registered Nurse Care Coordinator is responsible for the coordination, assessment, and delivery of patient-centered care. This individual will be responsible for developing individualized care plans in collaboration with patients, families, and other healthcare professionals to ensure the most comprehensive and effective care possible. Qualifications:• A current, valid RN license in the state of California • A minimum of 2 years of experience in a healthcare setting • Excellent written and verbal communication skills • A track record of delivering high-quality patient care • A commitment to providing compassionate, patient-centered care • The ability to work independently and as part of a team • Knowledge of nursing standards, processes, and regulations If you are passionate about providing quality healthcare and feel that you have the necessary qualifications and experience, we would love to hear from you!
