  1. Collaborate with transplant team to ensure smooth coordination of care for patients undergoing abdominal transplant procedures.
  2. Conduct thorough assessments of patients' physical, emotional, and psychosocial needs pre and post-transplant.
  3. Educate patients and families on the transplant process, including pre and post-operative care, medication management, and potential complications.
  4. Monitor patient progress and response to treatment, and adjust care plan as needed.
  5. Communicate regularly with physicians, nurses, and other healthcare team members to provide updates on patient status and ensure continuity of care.
  6. Serve as a patient advocate, addressing any concerns or issues that may arise during the transplant process.
  7. Coordinate and schedule necessary appointments and tests for patients, including donor evaluations and follow-up visits.
  8. Participate in multidisciplinary rounds and provide input on patient care plans.
  9. Maintain accurate and thorough documentation of all patient interactions and interventions.
  10. Provide emotional support and counseling to patients and families throughout the transplant journey.
  11. Stay current on advancements and changes in transplant medicine and incorporate new knowledge into practice.
  12. Adhere to hospital policies and procedures, as well as regulatory standards and guidelines.
  13. Collaborate with transplant team to develop and implement quality improvement initiatives.
  14. Act as a resource and mentor for other healthcare team members, including new staff and students.
  15. Continuously assess and improve processes to enhance the patient experience and improve outcomes.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing From An Accredited Institution.

  • Current And Active Rn License In The State Of California.

  • Minimum Of 3-5 Years Of Experience In A Transplant Or Critical Care Setting.

  • Certification In Transplant Nursing (Ctn) Or Solid Organ Transplant (Cst) Preferred.

  • Strong Communication, Organizational, And Critical Thinking Skills With The Ability To Work Independently And As Part Of A Multidisciplinary Team.
