
Pharmacy Technician - Transition of Care

- Collaborate with interdisciplinary team to review and process medication orders for patients transitioning between healthcare settings.
- Conduct medication reconciliation services to ensure accuracy and safety of patient's medication regimen.
- Assist in medication management for patients, including medication education and adherence support.
- Communicate effectively with patients, caregivers, and healthcare professionals to ensure seamless transitions of care.
- Maintain accurate and up-to-date patient medication records in compliance with regulatory standards.
- Adhere to all safety protocols and procedures when handling and dispensing medication.
- Participate in pharmacy quality improvement initiatives to enhance patient care and outcomes.
- Stay updated on current pharmacy practices and guidelines to ensure best practices are implemented.
- Provide exceptional customer service to patients and their families, addressing any concerns or questions.
- Maintain confidentiality and privacy of patient information at all times.
High School Diploma Or Equivalent: A Minimum Of A High School Diploma Or Ged Is Required For A Pharmacy Technician - Transition Of Care Position At Cedars-Sinai Medical Center. This Demonstrates A Basic Level Of Education And Knowledge Necessary For This Role.
Pharmacy Technician Certification: A Valid Pharmacy Technician Certification (Cpht) From An Accredited Institution Is Required For This Position. This Certification Ensures That The Candidate Has Received Proper Training And Has The Necessary Skills To Perform The Duties Of A Pharmacy Technician.
Previous Experience In A Pharmacy Setting: Prior Experience Working As A Pharmacy Technician In A Healthcare Environment, Particularly In A Transition Of Care Setting, Is Highly Desirable. This Demonstrates The Candidate's Familiarity With Pharmacy Operations And The Ability To Work Efficiently In A Fast-Paced Environment.
Strong Attention To Detail: As A Pharmacy Technician - Transition Of Care, It Is Essential To Have A High Level Of Attention To Detail To Accurately And Safely Dispense Medications. This Includes Verifying Prescription Information, Dosages, And Patient Information, As Well As Properly Labeling And Packaging Medications.
