
Critical Care Inpatient Clinical Pharmacist

- Provide comprehensive pharmaceutical care to critically ill patients in the inpatient setting.
- Collaborate with healthcare team to optimize medication therapy for patients.
- Conduct medication reconciliation and review to ensure safe and effective medication use.
- Monitor patient response to medication and make appropriate adjustments.
- Educate patients and caregivers on proper medication use and potential side effects.
- Participate in multidisciplinary rounds and provide drug information to healthcare team.
- Stay up-to-date on current treatment guidelines and pharmacotherapy advancements.
- Assess and identify drug-related problems and make recommendations for resolution.
- Oversee and ensure compliance with medication-related policies and procedures.
- Document interventions and medication therapy plans accurately.
- Communicate with physicians and other healthcare providers regarding medication therapy and patient progress.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Provide support and guidance to pharmacy technicians and other pharmacy staff.
- Maintain a safe and clean work environment in compliance with regulations.
- Continuously strive to improve patient satisfaction and overall quality of care.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.
Current Pharmacist Licensure In The State Of Montana.
Completion Of A Pgy2 Residency In Critical Care Pharmacy Or Equivalent Experience.
Board Certification In Pharmacotherapy Or Critical Care Pharmacy (Bcps Or Bcccp).
Minimum Of 2 Years Of Experience As A Clinical Pharmacist In An Acute Care Hospital, With At Least 1 Year In A Critical Care Setting.
