Social Work Case Manager

They should also have a minimum of two years of experience in a clinical setting and a valid driver's license. Responsibilities:

1. Provide social work services to patients and families in accordance with established organizational policies and procedures.
2. Create individualized treatment plans and coordinate care with other interdisciplinary team members.
3. Develop and maintain strong relationships with patients and families, providing emotional support and guidance.
4. Assess patients’ and families’ psychosocial needs and provide appropriate interventions.
5. Evaluate patient progress and update treatment plans as needed.
6. Follow up with patients and families to ensure ongoing progress and satisfaction.
7. Document patient care plans and progress in accordance with organizational policies and procedures.
8. Participate in regular team meetings to discuss patient progress and plan for future interventions.
9. Maintain current and valid license to practice social work in the state of Florida.
10. Maintain valid driver’s license and access to reliable transportation.
