Social Work Case Manager

Baptist Health South Florida is seeking a compassionate and motivated Social Work Case Manager to join our team. Our ideal candidate should have a passion for helping individuals and families in need by facilitating services and providing support. The Social Work Case Manager will provide care in the inpatient, outpatient or community setting, collaborating with an interdisciplinary healthcare team to ensure quality care for our patients.The successful candidate should have a Master’s degree in Social Work from an accredited university, along with a minimum of two years of experience in a clinical setting. A valid license in Social Work in the State of Florida is also required. Responsibilities:

1. Provide care and services in inpatient, outpatient, and/or community settings.
2. Work collaboratively with an interdisciplinary healthcare team to ensure quality care for patients.
3. Assist individuals and families in need by facilitating services and providing support.
4. Maintain a valid license in Social Work in the State of Florida.
5. Utilize strong interpersonal and organizational skills to work independently.
6. Conduct thorough research on community resources and services.
7. Participate in patient assessments and evaluations as needed.
8. Develop and update treatment and care plans.
9. Ensure adherence to laws, regulations, and ethical standards.
10. Provide education and counseling support to patients and families.
