- Conduct comprehensive assessments of patients' medical, social, and emotional needs.
- Develop individualized care plans in collaboration with patients, families, and interdisciplinary team members.
- Coordinate and facilitate communication and collaboration between patients, families, healthcare providers, and community resources.
- Monitor and evaluate patients' progress towards achieving their goals and make necessary adjustments to their care plans.
- Advocate for patients' rights and ensure their needs are met within the healthcare system.
- Educate patients and families on their care plans, disease processes, and self-management techniques.
- Utilize evidence-based practice and clinical knowledge to provide high-quality, cost-effective care.
- Document all patient interactions, assessments, and interventions accurately and timely.
- Participate in case management meetings and provide input on patient care and resource utilization.
- Stay current with industry trends and regulations related to case management.
- Collaborate with insurance companies to ensure appropriate coverage and reimbursement for patients.
- Provide support and guidance to less experienced case management staff.
- Maintain confidentiality and adhere to HIPAA regulations at all times.
- Participate in quality improvement initiatives and utilize data to improve patient outcomes.
- Continuously strive to improve the overall patient experience and satisfaction.
Bachelor's Degree In Nursing (Bsn) From An Accredited Institution.
Current And Valid Registered Nurse (Rn) License In The State Of Florida.
Minimum Of 2 Years Of Experience In Case Management, Preferably In A Healthcare Setting.
Knowledge And Understanding Of Medical Terminology, Procedures, And Documentation.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team.
