- Utilize clinical expertise to provide safe and effective medication therapy to patients within the healthcare organization.
- Collaborate with healthcare providers to optimize patient outcomes through medication management and monitoring.
- Conduct medication therapy reviews and make recommendations for therapy adjustments as needed.
- Identify and resolve medication-related problems and communicate with healthcare team to ensure appropriate patient care.
- Stay up-to-date on new medications, therapies, and treatment guidelines in the pharmaceutical field.
- Participate in interdisciplinary rounds and provide drug information to healthcare team.
- Provide education and counseling to patients and caregivers on proper medication use and potential side effects.
- Ensure compliance with all federal and state laws and regulations related to pharmacy practice.
- Maintain accurate and complete records of medication orders, dispensing, and patient profiles.
- Collaborate with pharmacy team to ensure efficient workflow and accurate medication dispensing.
- Assist in developing and implementing pharmacy policies and procedures.
- Participate in quality improvement initiatives and assist in maintaining accreditation and regulatory compliance.
- Serve as a mentor and resource for pharmacy staff and students.
- Participate in professional development activities and continuing education to enhance clinical knowledge and skills.
- Maintain a positive and professional attitude towards patients, healthcare team, and colleagues.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.
Current And Active State Pharmacist License In The State Of Florida.
Minimum Of 2-3 Years Of Experience As A Clinical Pharmacist In An Acute Care Hospital Setting.
Advanced Certification In Pharmacotherapy (E.g. Bcps) Or Specialized Training In Critical Care, Oncology, Or Other Relevant Specialty.
Strong Clinical Knowledge And Proficiency In Medication Therapy Management, Drug Interactions, And Formulary Management.
