As a Pharmacy Clinical Specialist, you will play a crucial role in ensuring the safe and effective use of medications for our patients.   1. Develop and implement medication therapy management plans for patients in collaboration with the healthcare team.
  2. Conduct comprehensive medication reviews and provide recommendations for appropriate drug therapy.
  3. Monitor medication therapy for effectiveness, adverse reactions, and drug interactions.
  4. Educate and counsel patients on proper medication use and potential side effects.
  5. Collaborate with physicians and other healthcare professionals to ensure safe and effective medication use.
  6. Participate in quality improvement initiatives to enhance patient care and safety.
  7. Stay updated on current pharmaceutical practices, new medications, and evidence-based guidelines.
  8. Provide drug information and assist with medication procurement and formulary management.
  9. Participate in medication reconciliation processes to ensure accurate and complete medication histories.
  10. Train and mentor pharmacy staff on clinical practices and procedures.
  11. Act as a resource for nursing staff and other healthcare professionals regarding drug therapy.
  12. Maintain accurate and timely documentation of all clinical activities.
  13. Participate in multidisciplinary rounds and provide recommendations for medication therapy optimization.
  14. Adhere to all state and federal laws and regulations governing pharmacy practice.
  15. Continuously evaluate and improve pharmacy processes to enhance patient care and improve outcomes.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution

  • Completed A Post-Graduate Residency Or Fellowship In Clinical Pharmacy Practice

  • Board Certification In A Relevant Specialty (E.g. Pharmacotherapy, Ambulatory Care, Critical Care)

  • Minimum Of 3-5 Years Of Experience As A Clinical Pharmacist In A Hospital Or Healthcare Setting

  • Strong Knowledge Of Current Pharmacy Laws, Regulations, And Best Practices In Patient Care And Medication Management.
