- Develop and implement medication therapy management plans for patients in collaboration with the healthcare team.
- Conduct comprehensive medication reviews and provide recommendations for appropriate drug therapy.
- Monitor medication therapy for effectiveness, adverse reactions, and drug interactions.
- Educate and counsel patients on proper medication use and potential side effects.
- Collaborate with physicians and other healthcare professionals to ensure safe and effective medication use.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Stay updated on current pharmaceutical practices, new medications, and evidence-based guidelines.
- Provide drug information and assist with medication procurement and formulary management.
- Participate in medication reconciliation processes to ensure accurate and complete medication histories.
- Train and mentor pharmacy staff on clinical practices and procedures.
- Act as a resource for nursing staff and other healthcare professionals regarding drug therapy.
- Maintain accurate and timely documentation of all clinical activities.
- Participate in multidisciplinary rounds and provide recommendations for medication therapy optimization.
- Adhere to all state and federal laws and regulations governing pharmacy practice.
- Continuously evaluate and improve pharmacy processes to enhance patient care and improve outcomes.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution
Completed A Post-Graduate Residency Or Fellowship In Clinical Pharmacy Practice
Board Certification In A Relevant Specialty (E.g. Pharmacotherapy, Ambulatory Care, Critical Care)
Minimum Of 3-5 Years Of Experience As A Clinical Pharmacist In A Hospital Or Healthcare Setting
Strong Knowledge Of Current Pharmacy Laws, Regulations, And Best Practices In Patient Care And Medication Management.
