Licensed Social Work Case Manager

Responsibilities:

1. Assist patients with care coordination services such as connecting them to appropriate resources, health care providers, and other support services.

3. Assist patients with financial, legal, and other social issues that impact their health.
4. Manage and monitor patient care plans to ensure they meet the needs of the patient.
5. Develop and maintain relationships with community resources and other health care providers.
6. Educate patients and their families on health care topics.
7. Maintain accurate records of client interactions and progress.
8. Follow up with patients to monitor their progress and ensure they are receiving the best possible care.
9. Monitor regulations and guidelines related to social work and health care to ensure compliance.
10. Participate in team meetings to discuss best practices and case management strategies.
