
Clinical Pharmacist, Pharmacy
We are looking for a highly motivated and experienced Clinical Pharmacist to join our Pharmacy team at Baptist Health South Florida. The Clinical Pharmacist will be responsible for providing comprehensive pharmaceutical care services while collaborating with healthcare providers to optimize patient outcomes. The ideal candidate is a licensed pharmacist with excellent clinical and communication skills. This individual must also be able to work collaboratively and demonstrate a commitment to providing patient-centered care. As part of our dynamic and hard-working team, the Clinical Pharmacist will be responsible for a variety of activities, including but not limited to: participating in medication order review; evaluating medication appropriateness; participating in collaborative practice agreements; attending daily rounds; providing advice and guidance on drug therapy; providing drug information; ensuring compliance with regulatory requirements; and providing medication counseling to patients. The successful candidate must possess a PharmD degree from an accredited college of pharmacy and be licensed in the state of Florida. In addition to excellent clinical and communication skills, the ideal candidate must demonstrate strong organizational skills, attention to detail, and the ability to work independently and as part of a team. The ability to troubleshoot problems and prioritize tasks is also essential.
Responsibilities:
- Review and evaluate medication orders for appropriateness and accuracy.
- Participate in collaborative practice agreements.
- Attend daily rounds and provide advice and guidance on drug therapy.
- Provide drug information to healthcare providers and patients.
- Ensure compliance with state and federal regulatory requirements.
- Provide medication counseling to patients.
- Maintain up-to-date knowledge of medications, drug interactions, and drug therapy.
- Troubleshoot problems related to drug therapies and prioritize tasks.
- Participate in professional development activities.
- Participate in performance improvement initiatives.
