  1. Collaborate with physicians, pharmacists, and other healthcare professionals to provide specialized care and support to patients receiving specialty medications.
  2. Conduct patient assessments and develop care plans to ensure safe and effective use of specialty medications.
  3. Educate patients and their families on the proper use and potential side effects of specialty medications.
  4. Monitor patient responses to specialty medications and communicate any concerns or changes to the appropriate healthcare team members.
  5. Maintain accurate and up-to-date patient records and documentation.
  6. Work with insurance providers to obtain prior authorizations for specialty medications.
  7. Collaborate with the pharmacy team to ensure timely and accurate dispensing of specialty medications.
  8. Participate in quality improvement initiatives to enhance patient care and outcomes.
  9. Stay informed on the latest developments and advancements in specialty medications and treatment options.
  10. Provide excellent customer service to patients, their families, and healthcare team members.
  11. Maintain patient confidentiality and adhere to all HIPAA regulations.
  12. Maintain compliance with all state and federal laws and regulations related to specialty pharmacy services.
  13. Participate in team meetings and contribute to the development of new processes and procedures.
  14. Foster a positive and collaborative work environment within the pharmacy team.
  15. Continuously strive to improve patient satisfaction and overall quality of care.

Job Qualifications
  • Bachelor's Degree In Pharmacy Or Related Field: A Strong Educational Background In Pharmacy Or A Related Field Is Essential For A Specialty Pharmacy Care Coordinator As They Will Be Responsible For Coordinating And Managing The Care Of Patients With Complex Medication Needs.

  • Pharmacist License In The State Of Kentucky: The Specialty Pharmacy Care Coordinator Must Be A Licensed Pharmacist In The State Of Kentucky In Order To Legally Dispense Medications And Provide Clinical Oversight To Patients.

  • Experience In Specialty Pharmacy Or Medication Therapy Management: Previous Experience In Specialty Pharmacy Or Medication Therapy Management Is Highly Preferred For This Role. It Demonstrates A Strong Understanding Of The Complexities Of Specialty Medications And The Ability To Provide Comprehensive Care To Patients.

  • Strong Communication And Interpersonal Skills: As A Central Pharmacy Coordinator, This Individual Will Be Working Closely With Patients, Physicians, And Other Healthcare Professionals. Strong Communication And Interpersonal Skills Are Essential For Effectively Coordinating Care And Building Relationships With All Stakeholders.

  • Knowledge Of Insurance And Reimbursement Processes: The Specialty Pharmacy Care Coordinator Will Be Responsible For Navigating Insurance And Reimbursement Processes For Specialty Medications. A Thorough Understanding Of These Processes Is Crucial For Ensuring Patients Receive Timely And Cost-Effective Care.
