Job Description

Baptist Health (Kentucky) is seeking an experienced and passionate Physical Therapist - Rehab to join our team. Our ideal candidate is an experienced physical therapist who is committed to providing exceptional rehabilitative services to our patients.The Physical Therapist - Rehab will be responsible for evaluating patients’ functional abilities, developing treatment plans, and providing instruction to patients and their families regarding the treatment plan. The therapist must also provide physical rehabilitation services to patients in accordance with the principles and practices of physical therapy.The successful candidate must possess an active physical therapist license in the state of Kentucky and have at least two years of physical therapy experience in a healthcare setting. Knowledge of evidence-based practice, strong communication skills, and the ability to build relationships with patients and colleagues are essential for this role. Responsibilities:

  1. Evaluate patients’ functional abilities and develop individualized treatment plans.
  2. Provide physical rehabilitation services to patients in accordance with the principles and practices of physical therapy.
  3. Educate patients and their families on the treatment plan and any home exercises as necessary.
  4. Monitor patient progress and modify treatment plans as necessary.
  5. Maintain accurate and up-to-date patient records.
  6. Maintain professional standards as outlined by the American Physical Therapy Association.
  7. Participate in continuing education opportunities to increase knowledge and skills.
  8. Utilize appropriate safety techniques while performing therapy services.
  9. Communicate effectively with patients, family members and other healthcare providers.
