  1. Develop and implement patient care plans in collaboration with the endocrinology team to ensure optimal management of endocrine conditions and medications.
  2. Conduct medication reviews and provide education to patients on proper medication use, potential side effects, and drug interactions.
  3. Serve as the primary point of contact for patients regarding their medications, including refills, prior authorizations, and insurance coverage.
  4. Collaborate with physicians, nurses, and other healthcare professionals to coordinate and streamline patient care.
  5. Monitor and track patient progress and medication adherence, making necessary adjustments to treatment plans as needed.
  6. Provide support and counseling to patients and their families on lifestyle modifications, including diet and exercise, to manage their endocrine conditions.
  7. Maintain accurate and up-to-date patient records, including medication profiles and treatment plans.
  8. Stay current on advancements in endocrinology and medication therapies to provide the best possible care to patients.
  9. Serve as an advocate for patients, ensuring their needs and concerns are addressed and addressed promptly.
  10. Participate in quality improvement initiatives and assist in developing policies and procedures related to endocrine care within the pharmacy department.
  11. Work with insurance providers to ensure timely and accurate processing of medication claims.

Job Qualifications
  • Bachelor's Degree In Pharmacy Or Related Field: The Pharmacy Care Coordinator - Endocrinology Position At Baptist Health Requires A Minimum Of A Bachelor's Degree In Pharmacy Or A Related Field. This Ensures That The Candidate Has A Strong Foundation In Pharmaceutical Knowledge And Can Effectively Manage Patient Medications And Treatment Plans.
