  1. Provide pharmaceutical care and expertise to patients within the healthcare system.
  2. Work with other healthcare professionals to ensure seamless transitions of care for patients.
  3. Conduct medication reconciliation and review to identify potential drug therapy problems.
  4. Develop and implement individualized care plans for patients to optimize medication therapy.
  5. Educate patients and their caregivers on proper medication use and potential side effects.
  6. Monitor patient response to medication therapy and make necessary adjustments.
  7. Collaborate with healthcare team to identify and resolve medication-related issues.
  8. Stay updated on current pharmaceutical trends and advancements in the industry.
  9. Maintain accurate and detailed patient records and documentation.
  10. Adhere to all state and federal laws and regulations related to pharmacy practice.
  11. Participate in quality improvement initiatives to enhance patient outcomes.
  12. Assist with training and mentoring of pharmacy staff.
  13. Attend and actively participate in team meetings and departmental initiatives.
  14. Maintain a positive and professional attitude while providing high-quality patient care.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.

  • Active And Unrestricted Pharmacist License In The State Of Kentucky.

  • Minimum Of 2 Years Of Experience As A Practicing Pharmacist, With A Preference For Experience In A Hospital Or Healthcare Setting.

  • Knowledge And Understanding Of Medication Therapy Management And Transition Of Care Processes.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Effectively Educate Patients On Medication Management.
