- Facilitate palliative care referrals: The primary responsibility of the Palliative Care Referral Nurse will be to coordinate and facilitate referrals for patients in need of palliative care services. Bachelor's Degree In Nursing And Current Rn License In The State Of Kentucky.
Minimum Of 2 Years Experience In Hospice Or Palliative Care Nursing.
Certification In Hospice And Palliative Care Nursing (Chpn) Preferred.
Strong Communication And Interpersonal Skills To Work Effectively With Patients, Families, And Healthcare Team.
Ability To Provide Emotional Support And Comfort To Patients And Families Facing End-Of-Life Care.
