Job Description

Baptist Health in Kentucky is looking for a talented and passionate Home Health Field LPN to join our team. As a Home Health Field LPN, you will be responsible for providing skilled nursing services and treatments in the home setting. You will be required to assess, plan, develop, implement, and evaluate the care of patients. You will also provide education and counseling to patients and their families.Qualifications:• Licensed Practical Nurse (LPN) in the state of Kentucky• Minimum of one year of experience in home health, medical/surgical, or long-term care preferred• Current CPR certification• Excellent communication, organizational, and problem-solving skills• Ability to think critically and make sound clinical judgments• Ability to work independently with minimal supervision• Ability to work in a fast-paced environment• Excellent customer service and interpersonal skillsIf you are an experienced and compassionate healthcare professional who is looking to make a difference in the lives of patients and their families, then this is the job for you!

Responsibilities: • Evaluate patient care needs and develop appropriate nursing care plans • Administer medications, treatments, and therapies as prescribed by the physician • Monitor patient progress and adjust treatments as needed • Provide education and counseling to patients and their families • Maintain accurate and timely documentation of patient care • Collaborate with other healthcare professionals to ensure optimal patient care • Maintain a safe, clean, and organized environment for patients • Participate in ongoing training and professional development activities • Follow all HIPAA regulations and other applicable laws and regulations
