Job Description

Baptist Health (Kentucky) is looking for an experienced and compassionate Cooperative Care Coordinator to join our team. Our ideal candidate will be a team-oriented and organized individual who is passionate about providing excellent patient care.The Cooperative Care Coordinator will serve as the central hub of communication between the patient, their family, and the healthcare team. This individual will be responsible for managing caseloads and providing support to patients and their families as they navigate the healthcare system. They will be responsible for understanding the patient’s needs, coordinating with other healthcare providers, and ensuring that the patient receives the best possible care.The ideal candidate will have a minimum of two years of experience in a healthcare setting, excellent communication and interpersonal skills, and a strong commitment to providing high-quality patient care. They must be able to work effectively in a fast-paced environment and manage multiple tasks at once. Responsibilities:

  1. Serve as the primary point of contact and central hub of communication between the patient, their family, and the healthcare team.
  2. Manage caseloads and provide support to patients and their families as they navigate the healthcare system.
  3. Understand the patient’s needs and coordinate with other healthcare providers to ensure the patient receives the best possible care.
  4. Maintain electronic health records and case management software.
  5. Develop and implement care plans and coordinate services for patients.
  6. Facilitate communication between patients, families, and healthcare providers.
  7. Monitor patient condition and progress, and report any changes or concerns to the healthcare team.
  8. Ensure all documentation is accurate and up-to-date.
  9. Provide emotional support and guidance to patients and their families.
  10. Respond to patient inquiries and requests in a timely manner.
