  1. Provide high-quality pharmaceutical care to patients by accurately and efficiently dispensing medications, monitoring drug therapy, and ensuring safe and effective medication use.
  2. Collaborate with healthcare team members to develop and implement comprehensive medication management plans for patients, taking into consideration their medical history, current health status, and treatment goals.
  3. Conduct medication reconciliation to identify and resolve any discrepancies or potential drug interactions.
  4. Educate patients on proper medication use, potential side effects, and any necessary precautions to ensure safe and effective treatment.
  5. Serve as a resource for healthcare providers and patients regarding drug information, dosing, and administration.
  6. Participate in multidisciplinary rounds and meetings to discuss patient progress and make recommendations for adjustments to medication therapy.
  7. Stay up-to-date on the latest developments and advancements in the field of pharmacy and incorporate new knowledge and technologies into practice.
  8. Maintain accurate and detailed patient records, including medication profiles and clinical notes.
  9. Adhere to all regulatory and safety standards, including proper storage and handling of medications.
  10. Act as a mentor and preceptor to pharmacy students and residents, providing guidance and support in their learning and development.
  11. Collaborate with other healthcare professionals to develop and implement policies and procedures related to medication management.
  12. Participate in quality improvement initiatives to continuously improve patient outcomes and ensure the highest level of pharmaceutical care.
  13. Serve as a patient advocate, promoting patient safety and rights in all aspects of medication use.
  14. Demonstrate a commitment to ongoing professional development through participation in continuing education programs and maintaining relevant certifications.
  15. Maintain a compassionate and professional demeanor while interacting with patients, families, and colleagues, fostering a positive and supportive work environment.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.

  • Current And Valid Pharmacist License In The State Of Kentucky.

  • Minimum Of 2-3 Years Of Experience As A Clinical Pharmacist In A Hospital Or Healthcare Setting.

  • Board Certification In A Relevant Specialty Area (E.g. Oncology, Critical Care, Etc.).

  • Strong Clinical Knowledge And Expertise In Pharmacotherapy, Drug Interactions, And Medication Management.
