- Provide specialized pharmaceutical care to patients with rheumatic conditions, including but not limited to medication counseling, drug therapy management, and patient education.
- Collaborate with the rheumatology team to develop and implement treatment plans for patients, taking into consideration their individual needs and medical history.
- Conduct medication reviews and ensure appropriate and safe use of medications for patients.
- Keep up-to-date with the latest developments and advancements in rheumatology and related fields to provide the most current and effective treatment options for patients.
- Monitor and manage medication therapy outcomes for patients, including identifying and addressing any potential drug interactions or adverse effects.
- Maintain accurate and detailed patient records and documentation, adhering to all relevant legal and ethical standards.
- Educate patients and their families on their medications, potential side effects, and proper administration techniques.
- Collaborate with other healthcare professionals, such as physicians and nurses, to provide comprehensive and integrated care for patients.
- Participate in quality improvement initiatives and contribute to the continuous improvement of pharmaceutical care services within the clinic.
- Maintain a high level of professionalism and compassion when interacting with patients, ensuring their comfort and satisfaction with their care.
- Adhere to all relevant laws, regulations, and ethical standards related to pharmacy practice.
- Proactively identify and report any potential risks or issues related to medication use to the appropriate healthcare team members.
- Stay current with insurance protocols and assist patients with insurance-related questions or concerns.
- Maintain inventory of pharmaceuticals and supplies, ensuring proper storage and handling of medications.
- Attend relevant conferences and training sessions to continuously improve knowledge and skills in the field of rheumatology pharmacy.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School
Active Pharmacist License In The State Of Kentucky
Board Certification In Pharmacotherapy (Bcps) Or Ambulatory Care Pharmacy (Bcacp)
Advanced Training Or Experience In Rheumatology And Rheumatology Medications
Excellent Communication And Patient Counseling Skills, With A Focus On Patient Education And Adherence To Treatment Plans.
