Job Description

We at Baptist Health (Kentucky) are looking for an experienced, skilled, and dedicated Case Management Analyst to join our team. We seek a professional with a passion for improving the lives of patients through the coordination of healthcare services. The ideal candidate for this position will possess a combination of technical and interpersonal skills, as well as a deep knowledge of care delivery systems and healthcare regulations. The Case Management Analyst will be responsible for developing and maintaining relationships with patients, their families, healthcare providers, and other stakeholders.The successful candidate must have a minimum of two years of experience in case management, health care or related field; a Bachelor's degree in health services administration, social work, or a related field; and a current RN or LCSW license. They must also have excellent communication, problem-solving, and organizational skills. A passion for helping people and a commitment to providing excellent customer service are essential qualities for this role.

Responsibilities:

  1. Develop and maintain relationships with patients, their families, healthcare providers, and other stakeholders.
  2. Utilize excellent communication, problem-solving, and organizational skills.
  3. Ensure compliance with healthcare regulations and policies.
  4. Monitor and evaluate patient care plans and ensure quality services to patients.
  5. Investigate and resolve patient issues in a timely manner.
  6. Develop strategies to improve care delivery systems.
  7. Generate reports and analyze data to inform decision making.
  8. Participate in professional development activities to stay abreast of latest trends in case management.
  9. Provide guidance and support to staff members.
  10. Collaborate with other departments to ensure continuity of care.

Compensation
