Job Description

  1. Conduct thorough assessments of patients' social and emotional needs, including any potential barriers to care.
  2. Develop and implement individualized treatment plans in collaboration with patients and their families, utilizing evidence-based practices and resources.
  3. Provide crisis intervention and support to patients and their families during times of crisis or transition.
  4. Connect patients and their families with appropriate community resources and support services to address their social and emotional needs.
  5. Collaborate with interdisciplinary healthcare team members to ensure that patients' medical and social needs are addressed in a comprehensive and coordinated manner.
  6. Maintain accurate and up-to-date documentation of patient interactions, treatment plans, and progress notes in accordance with organizational and regulatory standards.
  7. Participate in case conferences and team meetings to discuss patient progress and develop strategies for improving patient care.
  8. Advocate for patients' rights and collaborate with community partners to address social justice and health equity issues that impact patients' well-being.
  9. Provide education and support to patients and their families regarding coping strategies, self-care, and healthy lifestyle choices.
  10. Continuously assess and evaluate the effectiveness of interventions and treatment plans, making necessary adjustments to ensure the best outcomes for patients.
  11. Engage in ongoing professional development and stay updated on current trends and best practices in the field of social work.
  12. Promote a positive and inclusive work environment by fostering teamwork, respect, and open communication among colleagues and patients.
  13. Adhere to all ethical and legal standards related to social work practice, including maintaining patient confidentiality and obtaining informed consent.
  14. Participate in quality improvement initiatives and assist in developing and implementing programs and policies to improve patient care and outcomes.
  15. Demonstrate a commitment to the organization's mission, values, and vision, and actively contribute to creating a culture of excellence in patient care.

Job Qualifications
  • Master's Degree In Social Work (Msw) From A Council On Social Work Education (Cswe) Accredited Program.

  • Licensed Clinical Social Worker (Lcsw) In The State Of North Carolina.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In A Hospital Or Medical Center.

  • Experience Working With Diverse Populations And Knowledge Of Cultural Competency.

  • Strong Communication And Interpersonal Skills, Including The Ability To Work With Interdisciplinary Teams And Communicate Effectively With Patients And Families.
