  1. Act as the primary point of contact for patients and their families, providing them with a warm and welcoming experience at Atrium Health.
  2. Assess the needs of patients and their families and connect them with appropriate resources and support services.
  3. Collaborate with other healthcare professionals to develop and implement individualized care plans for patients.
  4. Maintain accurate and up-to-date records of patient interactions and resource referrals.
  5. Educate patients and their families on available resources and how to access them.
  6. Continuously monitor and evaluate the effectiveness of resource programs and make recommendations for improvement.
  7. Communicate with community organizations and agencies to stay informed about available resources and services.
  8. Provide emotional support and guidance to patients and their families during challenging times.
  9. Ensure compliance with all relevant policies, procedures, and regulations.
  10. Participate in training and professional development opportunities to enhance knowledge and skills.
  11. Serve as a positive ambassador for Atrium Health and represent the organization in a professional manner.
  12. Maintain confidentiality of patient information and adhere to HIPAA regulations.
  13. Foster a culture of compassion, empathy, and respect towards patients and their families.
  14. Stay current on industry trends and developments related to healthcare resources and support services.
  15. Perform other duties as assigned by the supervisor.

Job Qualifications
  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Diverse Teams And Stakeholders.

  • Bachelor's Degree In A Related Field Such As Healthcare Administration, Business Administration, Or Human Resources.

  • Minimum Of 2-3 Years Of Experience In Resource Management, Preferably In A Healthcare Setting.

  • Strong Organizational And Analytical Skills, With The Ability To Effectively Manage And Allocate Resources.

  • Knowledge Of Healthcare Regulations And Compliance, As Well As Proficiency In Relevant Software And Technology Systems.
