Registered Nurse - Program Coordinator

This position requires an individual with exceptional organizational, communication, and leadership skills to develop and support patient care initiatives. An ideal candidate must have a desire to improve patient experiences and outcomes and be able to effectively collaborate with multiple stakeholders. The ideal candidate should possess a minimum of a Bachelor’s degree in Nursing along with a current Registered Nurse license in the state of practice, and at least 5 years of experience in Nursing. We are looking for a Registered Nurse Program Coordinator who is self-motivated, organized, and an excellent problem solver. Responsibilities:

1. Develop and support patient care initiatives
2. Collaborate with multiple stakeholders
3. Improve patient experiences and outcomes
4. Lead and mentor other nurses
5. Ensure that all nursing practices are in accordance with state regulations
6. Maintain patient records and ensure accuracy of data
7. Create educational materials and develop protocols for patient care
8. Monitor patient satisfaction and evaluate patient outcomes
9. Design and implement quality improvement initiatives
10. Facilitate interdisciplinary communication and collaboration
11. Perform assessments to identify patient needs and provide appropriate interventions
12. Advocate for patients and families to ensure optimal care
13. Collaborate with physicians to develop an individualized plan of care
14. Attend continuing education and professional development courses as needed
