- Act as a health coach and advocate for patients, promoting and improving their overall well-being.
- Utilize evidence-based practices to provide education and guidance to patients on various health topics, including disease prevention and management, nutrition, and lifestyle modifications.
- Develop individualized care plans for patients to support their health goals and track progress.
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and comprehensive care for patients.
- Monitor and assess patient progress and make necessary adjustments to care plans as needed.
- Utilize effective communication skills to build rapport and trust with patients and their families.
- Document patient interactions and progress accurately and timely in electronic medical records.
- Stay updated on current healthcare trends, research, and best practices to provide the highest quality care to patients.
- Participate in training and development opportunities to enhance skills and knowledge in health coaching and patient care.
- Maintain confidentiality and adhere to ethical standards in all patient interactions.
- Adhere to all policies, procedures, and regulations of Atrium Health to ensure a safe and professional work environment.
- Serve as a positive role model for patients and colleagues by demonstrating professionalism, compassion, and empathy.
- Continuously evaluate and improve health coaching practices to provide the best possible care to patients.
- Act as a liaison between patients and healthcare providers to ensure effective communication and coordination of care.
Bachelor's Degree In Nursing Or Related Field: Atrium Health Requires All Registered Nurse Health Coaches To Have A Minimum Of A Bachelor's Degree In Nursing Or A Related Field. This Education Provides A Foundation In Healthcare And Prepares Individuals For The Role Of A Health Coach.
Active Rn License: Atrium Health Requires All Registered Nurse Health Coaches To Have An Active And Unrestricted Rn License In The State In Which They Will Be Working. This Ensures That The Health Coach Has Met The Necessary Requirements To Practice Nursing And Can Provide Safe And Effective Care To Patients.
Experience In Healthcare: Atrium Health Prefers Candidates With Prior Experience In A Healthcare Setting, Such As A Hospital, Clinic, Or Physician's Office. This Experience Provides A Knowledge Base In Healthcare Practices And Procedures, As Well As An Understanding Of Patient Care And Communication.
