  1. Coordinate and facilitate the admission and discharge process for patients.

Job Qualifications
  • Active Registered Nurse (Rn) License In The State Of Employment.

  • Minimum Of 2 Years Of Experience In A Hospital Setting, Preferably In A Transition Or Discharge Planning Role.

  • Strong Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Patients, Families, And Interdisciplinary Healthcare Teams.

  • Knowledge Of Healthcare Regulations And Reimbursement Processes Related To Admission And Discharge Planning.

  • Bachelor's Degree In Nursing (Bsn) Preferred, Or Willingness To Pursue Bsn Within A Designated Timeframe.
