
Population Health Patient Coordinator

- Coordinate care for patients: The primary responsibility of the Population Health Patient Coordinator will be to coordinate care for patients. This includes working closely with healthcare providers and other team members to ensure that patients receive the necessary resources and support to maintain their health.
- Maintain patient records: The coordinator will be responsible for maintaining accurate and up-to-date patient records. This includes inputting data into electronic health records, organizing and filing physical records, and ensuring patient information is kept confidential.
- Communicate with patients: The coordinator will serve as the main point of contact for patients, communicating with them about their care plans, appointments, and any necessary follow-up. This will involve both in-person and remote communication methods.

Bachelor's Degree In Healthcare Administration, Public Health, Or A Related Field.
