  1. Perform medication reconciliation for patients across transitions of care, including hospital admissions, transfers, and discharges.
  2. Review and verify patient medication lists for accuracy and completeness.
  3. Communicate with patients, caregivers, and healthcare providers to obtain necessary information for medication reconciliation.
  4. Collaborate with pharmacists and other healthcare team members to resolve any discrepancies or issues with patient medications.
  5. Document all medication reconciliation activities accurately and thoroughly.
  6. Maintain a strong knowledge of medication names, dosages, and indications to ensure accuracy during reconciliation.
  7. Adhere to all safety and regulatory guidelines in handling and documenting medications.
  8. Keep up-to-date with changes in medication therapies and regulatory requirements.
  9. Assist with medication inventory management and ordering as needed.
  10. Provide exceptional customer service to patients and healthcare providers.
  11. Act as a resource for patients and healthcare providers regarding medication information and inquiries.
  12. Participate in ongoing education and training to maintain and enhance skills and knowledge.
  13. Identify and report any medication errors or near-misses according to policies and procedures.
  14. Maintain patient confidentiality and adhere to HIPAA regulations.
  15. Actively contribute to a positive and collaborative work environment.
  16. Perform other duties as assigned by supervisors.

  • Certified Pharmacy Technician (Cpht) Or Equivalent Training And Experience
