Job Description

Atrium Health is seeking a compassionate and knowledgeable Patient Nurse Navigator to join our Teen Health department. In this role, you will play a vital role in advocating for and guiding teen patients through their healthcare journey.   1. Develop and maintain a comprehensive understanding of adolescent health, including physical, mental, and social aspects.
  2. Communicate effectively with teen patients and their families to assess their needs and develop a plan of care.
  3. Serve as a liaison between patients, families, and healthcare providers to facilitate coordinated and timely care.
  4. Provide education and support to patients and families on navigating the healthcare system and managing their health.
  5. Advocate for the unique needs and concerns of teen patients within the healthcare system.
  6. Collaborate with interdisciplinary healthcare team members to ensure comprehensive and patient-centered care.
  7. Monitor and track patient progress and outcomes, identifying and addressing any barriers to care.
  8. Stay up-to-date on current healthcare policies, regulations, and best practices related to adolescent health.
  9. Participate in quality improvement initiatives to enhance the delivery of care for teen patients.
  10. Maintain accurate and thorough documentation of patient interactions and interventions.
  11. Uphold patient confidentiality and privacy at all times.
  12. Continuously seek opportunities for professional development and growth to enhance knowledge and skills in adolescent healthcare.
  13. Build and maintain positive relationships with patients, families, and colleagues to foster a supportive and welcoming environment for teen patients.
  14. Adhere to all ethical and legal guidelines in the provision of care to teen patients.
  15. Collaborate with community organizations and resources to ensure patients have access to necessary support services.

Job Qualifications
  • Bachelor's Degree In Nursing Or A Related Field: A Strong Foundation In Nursing Principles And Practices Is Essential For A Patient Nurse Navigator - Teen Health, As They Will Be Responsible For Providing Direct Patient Care And Education.

  • Current Rn License: A Valid And Active Rn License Is Required For This Position, Ensuring That The Patient Nurse Navigator Is Qualified To Provide Medical Care And Make Clinical Decisions.

  • Experience In Adolescent Healthcare: Ideally, The Candidate Should Have Prior Experience Working With Teenage Patients And Addressing Their Unique Healthcare Needs. This Could Include Experience In Pediatric Nursing Or Working In A Specialized Teen Health Clinic.

  • Excellent Communication Skills: As A Patient Nurse Navigator, The Ability To Effectively Communicate With Teenage Patients And Their Families Is Crucial. This Includes Listening, Explaining Medical Information In A Clear And Understandable Manner, And Building Rapport And Trust With Patients.

  • Knowledge Of Community Resources: A Strong Understanding Of Local Community Resources, Such As Mental Health Services, Support Groups, And Educational Programs, Is Important For A Patient Nurse Navigator - Teen Health. They Will Need To Connect Patients With These Resources To Ensure They Receive Comprehensive Care.
