Nutrition Nurse Navigator

Responsibilities:

• Provide nutrition and health education to patients and their families • Coordinate care and services, such as food delivery, nutrition counseling, and nutrition education classes • Monitor patient progress and provide feedback and support • Assist with the development of individualized nutrition plans • Maintain accurate and up-to-date patient records • Collaborate with other healthcare professionals to ensure patient satisfaction • Act as a resource for patients and their families regarding nutrition and health • Answer questions and provide assistance to patients regarding nutrition and health care • Prepare reports and documents related to patient care • Participate in educational activities and community events related to nutrition and health
