Job Description

Are you a compassionate and driven social worker with a passion for making a positive impact in the lives of others? Do you have experience in discharge planning and a desire to work in a fast-paced healthcare setting? If so, Atrium Health is seeking a MSW Social Worker for our Discharge Planning team. In this role, you will play a vital role in coordinating the discharge process for patients, ensuring they receive the best care and support as they transition out of the hospital. We are looking for a highly motivated individual with a Master of Social Work degree and a strong understanding of healthcare systems. Join our dynamic team and help us provide exceptional care to our patients and their families.

  1. Coordinate and oversee the discharge process for patients, working closely with healthcare professionals and other stakeholders to ensure a smooth transition out of the hospital.
  2. Conduct comprehensive assessments of patients' social, emotional, and environmental needs to develop individualized discharge plans.
  3. Advocate for patients and their families to ensure they receive appropriate resources and support during the discharge process.
  4. Collaborate with interdisciplinary team members to address any barriers to discharge and facilitate timely and safe discharge planning.
  5. Provide counseling and emotional support to patients and their families, addressing any psychosocial needs or concerns.
  6. Utilize knowledge of healthcare systems and community resources to connect patients with appropriate services and support.
  7. Maintain accurate and up-to-date documentation of patient assessments, discharge plans, and interventions.
  8. Participate in multidisciplinary meetings and provide valuable input and recommendations for patient care and discharge planning.
  9. Stay current on social work best practices and trends in healthcare to provide high-quality and evidence-based care to patients.
  10. Demonstrate a strong commitment to ethical and professional standards while upholding patient confidentiality.
  11. Collaborate with other social workers and healthcare professionals to develop and implement quality improvement initiatives related to discharge planning.
  12. Assist in training and mentoring new social work team members, providing support and guidance to ensure their success.
  13. Serve as a liaison between patients, families, and healthcare professionals to facilitate effective communication and understanding.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University.

  • Active Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Of North Carolina.

  • Minimum Of 2 Years Of Experience In Discharge Planning Within A Healthcare Setting.

  • Knowledge Of Health Care Delivery Systems And Community Resources.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Multidisciplinary Teams And Effectively Advocate For Patients' Needs.
