- Coordinate and oversee the daily operations of the Diabetes Program, ensuring efficient and effective delivery of services to patients.
- Develop and implement program strategies and initiatives to promote healthy lifestyle choices and improve diabetes management for patients.
- Collaborate with healthcare providers and community partners to raise awareness and educate individuals about diabetes prevention and management.
- Provide support and guidance to patients, families, and caregivers on diabetes management, medication adherence, and lifestyle changes.
- Conduct assessments and evaluations to monitor the progress and outcomes of the program.
- Maintain accurate and up-to-date records of patient data, program activities, and outcomes for reporting and analysis.
- Stay informed about advancements in diabetes management and treatment to continually enhance the program's effectiveness.
- Assist in the recruitment, training, and supervision of program volunteers and staff members.
- Coordinate and participate in community outreach events to increase program awareness and engagement.
- Foster a positive and collaborative work environment within the team, promoting teamwork and open communication.
Bachelor's Degree In A Related Field Such As Public Health, Nursing, Or Healthcare Administration.
Minimum Of 3-5 Years Of Experience Working In A Healthcare Setting, Preferably With A Focus On Diabetes Management.
Knowledge Of Current Diabetes Treatment Guidelines, Industry Best Practices, And Emerging Trends.
Strong Leadership And Communication Skills, With The Ability To Effectively Collaborate With A Multidisciplinary Team.
Certification In Diabetes Education Or Management, Such As Certified Diabetes Educator (Cde), Preferred.
