- Provide compassionate and dedicated care to hospice patients and their families.
- Assist patients with daily living activities, including bathing, dressing, and grooming.
- Monitor and record patient vital signs and report any changes or concerns to the healthcare team.
- Administer medications and treatments as directed by healthcare professionals.
- Maintain a clean and safe environment for patients, following infection control and safety protocols.
- Communicate effectively with patients, families, and healthcare team members to ensure high-quality care.
- Provide emotional support and comfort to patients and their families during their end-of-life journey.
- Adhere to all state and federal regulations and guidelines for hospice care.
- Complete all necessary documentation accurately and in a timely manner.
- Participate in interdisciplinary team meetings and collaborate with other healthcare professionals to develop and implement individualized care plans.
- Attend required training and continuing education programs to maintain CNA certification and enhance knowledge and skills.
- Handle difficult or sensitive situations with empathy and professionalism.
- Maintain confidentiality and respect the privacy of patients and their families.
- Keep up-to-date with hospice care best practices and incorporate them into daily care.
- Perform other duties as assigned by healthcare team leaders.
