1. Ensure safe and effective medication management for patients by reviewing prescriptions, monitoring drug interactions, and providing medication counseling.
2. Collaborate with healthcare providers to develop individualized treatment plans for patients.
3. Conduct medication therapy management and provide recommendations for optimizing drug therapy.
4. Stay up-to-date on the latest developments in the field of pharmacy and implement evidence-based practices in daily work.
5. Educate patients and their families about their medications, including proper usage, potential side effects, and drug interactions.
6. Maintain accurate and complete patient records, including medication histories and drug therapy plans.
7. Participate in multidisciplinary rounds and provide clinical expertise to the healthcare team.
8. Work closely with pharmacy technicians to ensure accurate and timely dispensing of medications.
9. Adhere to all legal and ethical standards in pharmacy practice, including HIPAA and other regulatory requirements.
10. Continuously assess and improve pharmacy processes to enhance patient safety and satisfaction.
11. Provide excellent customer service to patients, healthcare providers, and other stakeholders.
12. Mentor and train pharmacy students and residents.
13. Attend continuing education programs and conferences to enhance knowledge and skills.
14. Collaborate with other pharmacists and healthcare professionals to develop and implement quality improvement initiatives.
15. Act as a resource for drug information for patients, healthcare providers, and other members of the healthcare team.
