Care Manager - RN

We are currently seeking an experienced and motivated Care Manager - RN to join our team and help us continue to provide excellent care. The ideal candidate will have a comprehensive understanding of clinical patient care and the ability to communicate effectively with patients, families, and other health care providers. The individual should demonstrate strong problem solving and analytical skills and be able to develop individualized patient care plans. As a Care Manager - RN, the individual is expected to work collaboratively with the multidisciplinary team to coordinate the patient’s care activities and ensure a positive patient experience. The successful candidate will have the following qualifications:• Bachelor’s Degree in Nursing or other health-related field• Current Registered Nurse (RN) license in the state of North Carolina• Minimum of 2 years' experience in a clinical care setting• Proven track record of providing excellent patient care• Ability to work in a fast-paced environment• Excellent interpersonal and communication skills• Familiarity with Microsoft Office programs, EMR systems, and other computer software

Responsibilities: • Develop and implement individualized patient care plans • Coordinate patient care activities with the multidisciplinary team • Monitor and evaluate patient progress and outcomes • Provide excellent patient care and ensure a positive patient experience • Educate patients and families on health care needs and treatments • Provide support and guidance to patients and families • Communicate with other health care providers to ensure continuity of care • Maintain accurate patient records and medical charts • Utilize electronic medical records (EMR) systems and other computer software • Adhere to all professional standards and regulations • Remain current on healthcare trends and best practices
