  1. Coordinate and manage the care of patients to ensure the delivery of high-quality healthcare services.
  2. Develop and implement individualized care plans for patients based on their specific needs and medical history.
  3. Collaborate with interdisciplinary teams to assess, plan, and implement patient care plans.
  4. Provide ongoing education and support to patients and their families regarding their healthcare needs and treatment plans.
  5. Monitor and evaluate patient progress, making necessary adjustments to care plans as needed.
  6. Facilitate communication between patients, families, and healthcare providers to ensure a seamless and efficient care experience.
  7. Advocate for patients and their needs, ensuring that they receive the best possible care and resources.
  8. Document all patient care activities accurately and thoroughly in accordance with organizational policies and procedures.
  9. Stay current on industry trends, best practices, and healthcare regulations to ensure the delivery of high-quality and evidence-based care.
  10. Participate in quality improvement initiatives and contribute to the development of new and innovative care approaches.
  11. Maintain a positive and professional attitude while working collaboratively with colleagues and other members of the healthcare team.
  12. Adhere to all ethical and legal standards in the provision of patient care and maintain patient confidentiality at all times.
  13. Continuously strive for personal and professional growth through ongoing education and training opportunities.
  14. Represent Atrium Health in a positive and professional manner at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing From An Accredited Institution

  • Active Registered Nurse (Rn) License In The State Of North Carolina

  • Minimum Of 2 Years Of Experience In A Clinical Or Community Health Setting, Preferably In Case Management Or Care Coordination

  • Certification In Case Management (E.g. Ccm, Acm) Or Willingness To Obtain Within 6 Months Of Hire

  • Strong Communication, Critical Thinking, And Organizational Skills, With The Ability To Work Collaboratively With Interdisciplinary Teams And Diverse Patient Populations
