- Provide case management and discharge planning services to patients and their families.

- Utilize strong clinical expertise to assess and identify patients' needs and coordinate appropriate resources.
- Communicate effectively with patients, families, and interdisciplinary teams to ensure a smooth transition from hospital to home.
- Serve as a patient advocate and ensure that their physical, emotional, and social needs are addressed.

Valid Rn License: A Registered Nurse Who Specializes In Case Management And Discharge Planning At Ascension Health Must Hold A Current And Valid Rn License In The State In Which They Will Be Practicing.
Bachelor's Degree In Nursing (Bsn): A Bsn Is Often Preferred For This Role As It Provides A Strong Foundation In Nursing Theory And Practice, As Well As Leadership And Critical Thinking Skills Necessary For Effective Case Management.
Case Management Certification: Many Employers, Including Ascension Health, Prefer Or Require Their Case Management Nurses To Hold A Certification In Case Management, Such As The Certified Case Manager (Ccm) Credential.
Experience In Case Management And Discharge Planning: While Specific Experience Requirements May Vary, Most Employers Will Look For Candidates With At Least 2-3 Years Of Experience In Case Management, Preferably In A Healthcare Setting.
