  1. Develop and implement individualized care plans for patients, working closely with interdisciplinary team members to ensure the best possible outcomes.
  2. Conduct thorough assessments of patients' physical, emotional, and social needs to identify potential barriers to care and develop appropriate interventions.
  3. Collaborate with healthcare providers, insurance companies, and community resources to coordinate and optimize patient care.
  4. Educate patients and their families on healthcare options, resources, and self-care techniques to promote effective self-management of their conditions.
  5. Utilize critical thinking skills to assess, prioritize, and manage multiple patients' needs simultaneously.
  6. Maintain accurate and up-to-date documentation of patient care, including progress notes, treatment plans, and discharge plans.
  7. Advocate for patients' needs and rights, ensuring they receive the best possible care and have access to necessary resources.
  8. Attend and actively participate in team meetings, sharing knowledge and expertise to improve patient outcomes.
  9. Remain current on industry trends, best practices, and regulations related to case management and healthcare in general.
  10. Serve as a mentor and resource for other team members, providing guidance and support as needed.
  11. Ensure compliance with all regulatory and accreditation standards, as well as organizational policies and procedures.
  12. Continuously evaluate and improve processes and procedures to enhance the quality and efficiency of patient care.
  13. Foster a positive and collaborative work environment, promoting teamwork and open communication among team members.
  14. Maintain confidentiality and adhere to ethical standards in all aspects of job performance.

Job Qualifications
  • Current Rn License: Candidates For A Registered Nurse - Case Management Job At Ascension Health Must Hold A Valid And Current Registered Nurse License In The State Where The Position Is Located.

  • Bachelor's Degree In Nursing: A Minimum Of A Bachelor Of Science In Nursing (Bsn) Degree Is Typically Required For This Role. This Degree Provides A Strong Foundation In Clinical Skills And Evidence-Based Practice, Which Are Essential For Case Management.

  • Clinical Experience: Ascension Health May Require Candidates For This Position To Have A Minimum Of 2-3 Years Of Clinical Experience As An Rn. This Experience Should Include Direct Patient Care And Exposure To A Variety Of Healthcare Settings.

  • Case Management Certification: While Not Always Required, Candidates With Certification In Case Management (Such As Ccm Or Acm) Will Have A Competitive Edge. These Certifications Demonstrate A High Level Of Proficiency And Understanding In Case Management Principles And Practices.

  • Strong Communication And Organizational Skills: Case Management Requires Excellent Communication Skills, Both Written And Verbal, To Effectively Collaborate With Patients, Families, And Healthcare Teams. Additionally, Organizational Skills Are Crucial For Managing Multiple Cases And Coordinating Care Plans For Patients.
