Licensed Practical Nurse - Primary Care

1. Provide compassionate and high-quality care to patients in accordance with Ascension Health's mission and values.
2. Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans.
3. Conduct patient assessments, monitor vital signs, and administer medications as prescribed.
4. Maintain accurate and up-to-date patient records and documentation.
5. Educate patients and their families on disease management, health promotion, and preventative care.
6. Communicate effectively with interdisciplinary team members to coordinate patient care and ensure continuity of care.
7. Adhere to infection control and safety protocols to maintain a safe environment for patients and staff.
8. Participate in quality improvement initiatives to enhance patient care outcomes.
9. Stay updated on current healthcare practices, policies, and procedures through ongoing education and training.
10. Serve as an advocate for patients, particularly those who are vulnerable and in need of special attention.
11. Demonstrate professionalism and ethical conduct in all interactions with patients, colleagues, and the community.
12. Maintain a positive and collaborative work environment within the Primary Care team.
13. Uphold all state and federal regulations and licensing requirements for LPNs.
14. Continuously assess and evaluate patient needs and provide appropriate interventions.
15. Participate in on-call rotations and respond to urgent patient needs as required.
