Job Description

  1. Provide high-quality and compassionate patient care in a primary care setting.

  3. Monitor and assess patient's health status and report any changes to the healthcare team.
  4. Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
  5. Educate patients and their families on disease management, treatment plans, and self-care techniques.
  6. Ensure accurate and timely documentation of patient information and care provided.
  7. Adhere to all safety protocols and infection control measures.
  8. Maintain a clean and organized work environment.
  9. Participate in quality improvement initiatives and assist in identifying areas for improvement.
  10. Serve as a patient advocate and communicate any patient concerns or feedback to the healthcare team.
  11. Stay updated on current nursing practices and regulations to provide the best possible care.

  13. Work collaboratively with other members of the primary care team to provide comprehensive care to patients.

Job Qualifications
  • Valid Lpn License: The Candidate Must Possess A Valid And Current License As A Licensed Practical Nurse In The State Where The Job Is Located. This License Must Be In Good Standing And Free Of Any Disciplinary Actions.

  • Minimum Education Requirements: The Candidate Must Have Completed An Accredited Practical Nursing Program And Hold A Diploma Or Associate Degree In Nursing.

  • Experience In Primary Care Setting: The Ideal Candidate Should Have At Least 1-2 Years Of Experience Working As An Lpn In A Primary Care Setting. This Experience Should Include Providing Direct Patient Care, Administering Medications, And Assisting With Procedures.

  • Excellent Communication Skills: As An Lpn In A Primary Care Setting, The Candidate Will Be Expected To Communicate Effectively With Patients, Families, And Other Healthcare Professionals. Therefore, Strong Verbal And Written Communication Skills Are Essential.

  • Knowledge Of Electronic Medical Records (Emr): Ascension Health Utilizes Electronic Medical Records For Their Patient Documentation. The Candidate Should Have Experience And Proficiency In Using Emr Systems To Document Patient Care And Manage Patient Information.
