
Field Behavioral Health Case Manager

- Provide comprehensive case management services to patients with behavioral health needs.

- Stay informed about current trends and best practices in behavioral health.

- Maintain confidentiality and adhere to ethical standards in patient care.
- Identify and address barriers to treatment and develop solutions to improve patient outcomes.
- Participate in training and development opportunities to continuously improve skills and knowledge.

- Work collaboratively with community organizations and resources to support patient needs.

Bachelor's Degree In Social Work, Psychology, Counseling Or Related Field.
Minimum Of 2 Years Experience In Case Management, Social Work Or Counseling.
Knowledge Of Behavioral Health Resources, Treatment Options And Community Support Programs.
Excellent Communication And Interpersonal Skills To Work With Diverse Populations And Build Rapport With Patients.
Ability To Work Independently And As Part Of A Multidisciplinary Team To Develop And Implement Treatment Plans For Patients.
