  1. Conduct thorough assessments of patients' behavioral health needs and develop individualized care plans in collaboration with the patient, their families, and other healthcare professionals.
  2. Provide ongoing support and guidance to patients in managing their behavioral health conditions, including coordinating appointments, referrals, and resources.
  3. Monitor and document patients' progress and treatment outcomes, ensuring accurate and timely reporting.
  4. Collaborate with interdisciplinary teams to develop and implement strategies to improve patient outcomes and quality of care.
  5. Maintain knowledge of current trends and best practices in the field of behavioral health and apply them to patient care.
  6. Advocate for patients' rights and needs, ensuring they receive appropriate and timely care.
  7. Coordinate and facilitate communication between patients, families, healthcare providers, and community resources.
  8. Participate in regular team meetings and case conferences to discuss patient progress and develop care plans.
  9. Provide crisis intervention and support to patients in acute situations.
  10. Maintain accurate and up-to-date documentation of patient interactions and treatment plans.
  11. Adhere to all ethical and legal standards in the provision of services and maintain confidentiality of patient information.
  12. Collaborate with insurance companies and other payers to ensure appropriate coverage and reimbursement for services.
  13. Continuously evaluate and improve processes and procedures to enhance the quality and efficiency of care provided.
  14. Act as a liaison between the organization and community resources, building and maintaining relationships to improve patient outcomes.
  15. Uphold the mission and values of Ascension Health in all interactions and responsibilities.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Counseling, Or A Related Field.

  • Minimum Of 2 Years Experience In Behavioral Health Case Management Or Related Field.

  • Knowledge And Understanding Of Mental Health And Substance Abuse Disorders, Treatment Options, And Community Resources.

  • Excellent Communication And Interpersonal Skills, Including The Ability To Build Rapport With Clients And Collaborate With Other Healthcare Professionals.

  • Strong Organizational Skills And Ability To Manage A Caseload, Prioritize Tasks, And Meet Deadlines.
